Dear Doctor,
Re: Mr Timothy Patterson, aged 69 Years
I am referring Mr Patterson, who was diagnosed with a lower left arm laceration, and is alleged orthostatic hypertension, for your complete cardiac monitoring and further care.
Mr Patterson has had hypertension since 2008 and is non-compliance with exercise as well as medication. Apart from this, he is following a sedentary life style, and is having excess body weight compared to his height.
On 01 July 2019, Mr Patterson was admitted to the hospital with a deep lower arm wound. After the assessment, he was initiated with intravenous antibiotics and wound care. Subsequently, the antibiotic was ceased and oral antibiotic was commenced. Currently, excellent performance of the wound is noted. However, his blood pressure has been severely elevated. His current pressure was recorded as 190/84mmhg, 196/86mmhg while sitting and supine position; respectively.
Pertaining to the above-mentioned information, kindly take over the case of Mr Patterson and provide your expert assessment and further evaluation. Complete cardiac observation and further blood pressure verification would be profoundly beneficial. All the previous blood pressure report is attached with this letter for your kind review.
If you require any additional information, please do not hesitate to contact me at any time.
Yours sincerely,
Amber Diva
