Re: Ms Sona Mittal, aged 2 Years


I am referring Ms Sona, who has been malnourished, requires continuous nutritional management, following her discharged today.


Ms. Sona lives with her unemployed mother and is staying in a non-completed house. Her father lives abroad and cannot travel back to home due to travel restriction.


On 20 October 2019, Ms Sona was admitted to the hospital with complaints of dyspnea, cough, nutritional deficiency, and other associated ailments. After the assessment, dry skin, sunken eyes, and low body weight were noticed. Therefore, antibiotics and steam inhalations were commenced on her. Additionally, her nutritional needs have been settled during hospitalization. Excellent improvement of the child's condition has been noticed. However, sleep disturbance was noted, and for which affect her mother's sleep.


Pertaining to the above-mentioned information, kindly take over the case of Ms Sona and provide ongoing nutritional support. Educating her mother regarding the preparation of nutritional meal and compliance with prescribed antibiotics and vitamin C tablets for the next one week would be profondly beneficial. She is scheduled for a nutritionist as well as speech-therapist follow-up on 01 November 2019.
