Re: Mr. Alfred Billy, Aged 52

Iam writing this letter to refer Mr Billy who reguirn daily observation and care followed by ccb removal

stergery He is being discharged today.

On 21 March 2010 he was admitted and skin biopsy was performed for pathological study. he was

diagnosed with skin cancu, basel celll carcinoma, neck, nodulas basat cell carcinoom. Hence, he

undergone this surgery.

Post-operatively, on assessment mild wound infection, breakdown suture reactions were identified.

Additionally, suspected about incomplete crccision. On following, normal heating process, no persistant

swelling were noted, commenced with panadein forte 500 mg for pain relies.

He is medically free and a truck driver. He has habits of smoking and drinking. He is married but lives

seperatly. Kindly note that, emergancy address and current address were attached with this letter.

Upon discharge, he is perfectly well and does not complaint any pain, no complications were noted. He

requires daily observation. Please note, he should continue the medications for one more week.
