Re: Mr Lionel Ramamurthy,aged 63

I am writing to update the current health status of Mr Lionel Ramamurthy, who has diagnosed with pneumonia,requires continuing care and dietry management.He has been discharged tomorrow.

On 04 th of this month, Mr Ramamurthy was admitted with the symptoms suggestive of pneumonia.In addition,he has shortness of breath,chest and abdominal pain associated with persistant cough.On admission,he has very weak and ambulate short distance with pick-up frame.He was assisted with activites of daily living.

During hospitalization,he become independ in ambulation,showering and the usage of toilet.Stil he have mild chest and abdominal pain due to dry cough.Apart from this,he was reviewed by a dietitian and gained weight of 1.5kg.

It would be benifical if you could monitoring Mr Ramamurthy’s nutrtion;which includes increased oral fluids intake,eggs and fruits.Physiotherapy has been initated and encourage him to do deep breathing and coughing exercise.Kindly, ensure that sitting position preferred than lying down to ensure postural drainage.It is important to keep him warm and take paracetamol as required for pain.
