Re: Mr Jim Middleton, aged
This letter is being written to refer Mr Jim Middleton, who underwent surgical repair of inguinal hernia, requires an appropriate assistance from your facility at home following the discharge today.
On 7 July 2008, Mr Middleton admitted with the plan of elective surgery. Post-operative period was uneventful exception of
minimal pain on movement. Therefore, Panadol two tablets six hourly has been commence
Mr Middleton lives with his wife in their own home with a large garden which they maintain independently. They never applied for pension or home assistance as they are financially independent, but Mrs Middleton is worried as investments have reduced due to stock market falls. They are concerned of not being able to maintain the garden or afford a gardener at present.
Mr Middleton was instructed to refrain from driving for at leat six weeks, avoid heavy lifting, but limited to light exercises. Additionally, they have poor access to public transportation, therefore they have to relay on taxis as appointment with surgeon an 11 August at 10:00am has been scheduled.
Based on the aforementioned circumstances, evaluation of situation for eligibility to receive a pension and home care would be highly recommend. Of note, Mrs Middleton prefers a home visit because she has hearing difficulties, for which phone conversation is not suitable. The address is enclosed with this letter.
