Dear Nurse,
Re: Ms Betsie Anderson, aged 89
I am referring Ms Betsie Anderson, who was diagnosed with urinary tract infection, for ongoing care and further assistance at your facility, following the discharge back today.
On 16 February 2020, Ms Anderson was admitted with the complaints of urinary incontinence along with low grade fever. She commenced on tablet acetaminophen 650mg orally and injection ciprofloxacin intravenously.
Drowsiness and sleepiness were noticed on admission. On further investigation, elevation of the white blood cells and presence of bacteria in the urine was reported.
Ms Betsie Anderson has had history of hypothyroid, dementia, constipation, gastric reflux disease and undergone a right hip surgery in 2017. Tablet levothyroxine 200mcg, protonix 75mg daily, colace 100mg morning, ciprofloxacin 250mg orally after 12 hours for fourteen days and acript10mg in evening time were prescribed.
Subsequently, Ms Anderson's condition has been improved with reduction in solomence and disorientation. Assistance provided in activities of daily living and Physiotherapy. Appropriate hygiene with frequent toileting is ensured. Regular assessment of vital signs and loss of consciousness are suggested.
On the basis of aforementioned circumstances, a follow-up visit with Dr Alex after 2 weeks with repeat urine is scheduled. Additionally, cane, walker and commode are recommended for ambulation. Regular monitoring for the signs of dehydration and orthostatic blood pressure should be organised.
