Dear Nurse,

Re: Mr Robert Jai, aged 70

Mr Robert Jai, who is recuperating from his appendectomy surgery, requires unceasing care and assistance following his transfer today.

On 10 May 2019, Mr Jai presented to the hospital with complaints of severe abdominal pain. Ultrasonography result revealed inflamed appendix for which he underwent surgery on the second postoperative day, oozing was noticed from the wound and wound edges were red in colour. Moreover dressing were performed daily.

Mr Jai lives alone. Habit of smoking was reported for 30 years and allergic to codeine. Right side paralysis and slurred speech was reported after cerebrovascular accident which takes place in 2018. Underwent tonsillectomy last year. He has had diabetes and hypertension for 10 years. His current medications include metformin 500mg, alphadopa 250mg.

Based on the aforementioned circumstances, it would be profoundly beneficial if you could provide assistance with showering and activities of daily living. Regular periodic wound dressings are recommended. Abundant fluid consumption is advised. Soft diet is necessary until he got recovered from illness. Regular physiotherapy exercises are requested to a visit with mobilization. Regular monitoring of blood glucose levels, medication compliance if recommended.
