Re: Mr Gerald Baker ;aged 79
I m writing to update regarding Mr Baker who was admitted on 30/09/2018 and underwent a left total hip replacement.He is being discharged back into your faculty and requires continued care
During hospitalization , his daily dressings were done on the incision site and pain relief was commenced to him .He was suggested range of motion exercises to improve his mobility .In addition , occupation therapy was started .
Post operatively, he appeared disoriented which may be related to the anaesthesia and dropped Hb level to 72g/dl ;therefore , he was transfused three units of blood.Upon discharge, his Hb level was stable and needs to be monitored continuously.
Mr Baker has good mobility and can walk along the ward using a wheel-walker without difficulty.He has been recommended aspirin 100mg and advised to continue his previous medications .
Given the above, it would be greatly appreciated if you continue monitor his medications and preserve skin integrity .it is important to encourage him for daily exercise programme .wevare sending a walker and wedge pillow with him .our social worker has organised hire of a toilet raiser for two weeks .Please note , he has an appointment in two weeks in order to remove his staples .Also , he has follow FBE and UEC tests at hospital.
