Dear Doctor,
Re: Ms Helen Marshal, DOB: 29 January 1968
I am referring Ms Helen Marshal, who has been provisionally diagnosed with myocardial
infarction for immediate assessment and appropriate management.
Presently Ms Marshal has been complaining of chest pain along with chest tightness which
radiates to the shoulder and neck areas with the rate of eight out of ten a pain scale. The pain was
not responding either to the Panadol or buscopan. Her blood pressure also has elevated
-190/100.Subsequently, not only the Nitroderm transdermal patch ,but nasal prong oxygen two
liters also has been commenced.
Today, Ms Marshal was suffering from general weakness and tiredness since 1945 hours, which
was informed to the General Practitioner. However, she was not visited due to the busy schedule
but pain relievers were prescribed.
Ms Marshal has had hypertension from ten years, for which she takes captopril twenty-five
milligrams orally twice daily. Even though her alcohol consumption is less, she smokes one pack
of cigarettes daily.
Based on the aforementioned circumstances, urgent consultation and effective treatments would
be extremely beneficial to manage Ms Marshal’s health condition.
If any additional information is required, please do not hesitate to contact me.
Yours faithfully,
