Re: Mrs Jennifer Milton, age 35
Thank You for accepting Mrs .Jennifer Milton , who is 25 week’s antenatal mother. Mrs . Milton is been referred to you for expert advice and management for hyperglycaemia and hyperthyroidism.
During hospitalisation, the blood sugar level was remarkable simultaneously with the thyroid profile. TSH was 6mU/L where as random blood sugar was 300mg/dl. According to the history stated, Mrs. Milton’s father is also a known diabetes.
Throughout the stay, urine and blood sugar were monitored. A refer to the dietician and the nutritional assessment was also done respectively. Metformin 500 mg twice daily was commenced. Education regarding dietary modification was also taken into consideration.
Slight decreased in weight was detected prior to the admission by almost 8 kg. During the entire hospital stay, no further weight loss was recorded.
