Re:Mrs Anne Ellis;DOB:06 January,1942
I am writing to update current condition of Mrs Ellis, who has been diagnosed with renal colic.She requires continued care and management after being discharged today.
As you already aware Mrs Ellis was admitted to hospital due to multiple complaints on 12 June 2018.On examination CT scam revealed bilateral multiple calculi,moderate bilateral hyderonephrosis, and dilation of left collecting pelviureteric junction calculus.consequently,she was commenced on ivfluids,pain killer,and antiemetic.
Currently,she is started tamsolisin8 mg bd and paracetamol 500 mg 4-6 hourly .on urology assessment stone will pass spontaneously in 2-3 weeks. Currently she is fit to discharged. She has fixed an appointment with urologist on 20 July.
In view of the above, it would be greatly appreciated if you could do monitoring of compliance of medication, urine output and increase fluid intake. Please note use sieve to check passage of stone and send to lab for investigation. It is important to intervention by dietician for decrease salt intake ,avoid excessive intake of Ocala ye rich food, and limit animal protein according to body weight.
