Re: Ms Sonia Calleja, aged 85
I am reffering ms sonia calleja, who was diagnosed with right inguinal hernia , require continue ongoing care and social support , following her discharge today.
On 28 August 2018, Ms Calleja was reffer to the emergency department due to sudden abdominal pain, nausia and constipation, concequently , she underwent right inguinal hernia repairs. post operatively she has remarkable progresis. Mobilization was initiated , prove to fall, hence four wheel frame walker was suggested , after assessment had done by geriatric . Rehabilitation unit, she is anxious and not happy to go back to independent living unit after discharge from the hospital therefore , aged care assessment team had recommended to be transfer to the huntington tower , for continue monitoring under level of nursing care.
Ms calleja lives in an independent living unit alone , which is close to her daughter, she is chain smoker.
Based on above afore mentioned circumstances . It would be profoundly appericiated if you could provide support to ms calleja and her family regarding transition to an aged care, espically her daughter she is frustrated , not able to help her mother by any means.
